- The person is deceased: PLEASE DO NOT CALL THE JURY OFFICE TO PROVIDE THIS INFORMATION. Please respond either online via eJuror or in writing. If you choose to respond in writing, please write a note on the front of this letter stating (1) that the person is deceased, (2) your relationship to the deceased, and (3) your signature. Return the letter to the Court at: U. S. District Court, ATTN: Jury Office, 1729 5th Avenue North, Birmingham, AL 35203.
- You no longer live in the Northern District of Alabama: The 31 counties that make up the Northern District of Alabama are: Bibb, Blount, Calhoun, Cherokee, Clay, Cleburne, Colbert, Cullman, DeKalb, Etowah, Fayette, Franklin, Greene, Jackson, Jefferson, Lamar, Lauderdale, Lawrence, Limestone, Madison, Marion, Marshall, Morgan, Pickens, Saint Clair, Shelby, Sumter, Talladega, Tuscaloosa, Walker, and Winston County. If you no longer live in one of these counties, you may correct your address online via eJuror choosing “Other,” as your County of Residence. Continue to respond to the remaining questions as appropriate. Or you may return this letter with a note reflecting your new address. You will be disqualified from serving as a juror in this district.
- You are 70 years of age or older and do not wish to serve as a juror: Please complete your questionnaire online via eJuror (there is a specific question on the online questionnaire that will give you an opportunity to request an excuse based on your age), OR you may write a note on the front of this letter stating (1) your date of birth, (2) the fact that you do not wish to serve as a juror and (3) your signature. Return the letter to the Court at: U.S. District Court, ATTN: Jury Office, 1729 5th Avenue North, Birmingham, AL 35203. PLEASE NOTE: For those individuals 70 years of age or older who DO wish to serve, we welcome and appreciate your willingness!
- You are unable to read, write, speak, or understand the English language OR have amental or physical disability that would prevent you from completing the questionnaire or serving as a juror in this court: For individuals who fall into this category, a family member or friend can assist in completing the questionnaire; however, the person completing the form should answer No to the online question “Are you, JUROR’S NAME, filling out this form?” on the Welcome screen. Respond to the remaining questions as appropriate.
At this point in the process, the only excuse requests that will be accepted are those covered above. ALL OTHER REQUESTS MAY BE MADE AT THE TIME YOU RECEIVE A SUMMONS FOR JURY SERVICE.
You must complete the questionnaire regardless of your age, health, place of residence, availability to serve, or any other factor.
Please complete the questionnaire online through our electronic jury system, eJUROR. If you cannot or do not wish to submit the questionnaire in electronic format, a paper questionnaire will be mailed to you soon. You may fill it out and return it to the court in the prepaid envelope provided. If you complete the questionnaire online, you do not need to return the paper questionnaire.
Do not call or send letters or emails asking to be excused from jury duty at this time. If you are summoned later, you will have the opportunity to request to be excused then.
Our system of justice is founded on the right to a trial by jury. Jury service is both a privilege and an obligation of every American citizen. FAQ About the Jury Wheel Refill
- Am I being summoned? If so, why?
If you received a JQQ Letter, you are not being summoned at this time. This is only a request for you to complete the questionnaire to see if you are qualified for potential future jury service. Filling out the questionnaire does not mean that you will necessarily ever be summoned for jury duty. It is very important that you complete the questionnaire online within 10 days.
- What if I don’t have a computer?
Please complete the questionnaire online if you can. If you are not able to do so, please call 1-844-466-7217, select Option 1 then Option 1 again, and enter your participant number to request a paper copy be mailed to you. The 9-digit participant number can be found above your name and address on the JQQ Letter you received.
- I’m 70 or over, do I have to serve?
You get to choose if you wish to serve. Please complete your questionnaire online via eJuror (there is a specific question on the online questionnaire that will give you an opportunity to request an excuse based on your age), OR you may write a note on the front of this letter stating (1) your date of birth, (2) the fact that you do not wish to serve as a juror and (3) your signature. - I have a medical condition and can’t serve. What do I do?
If you have a medical condition that you feel prevents you from serving, please do not submit proof or documentation. Simply complete the questionnaire online as requested. If you are summoned in the future, you will have an opportunity to submit documentation at that time. Any documentation received at this stage will not be processed or reviewed.
- I have an occupation or religious belief that prevents me from serving. What do I do?
If you have an occupation or religious belief that you feel prevents you from serving, you should still complete the questionnaire online as requested. If you are summoned in the future, you will have an opportunity to submit documentation at that time. Any documentation received at this stage will not be processed or reviewed.
- I have moved or do not live in the Northern District of AL. Do I still have to serve?

If you are the primary caregiver for a child under the age of 10, or an aged or infirmed person and feel that you are unable to serve, you should still complete the questionnaire online as requested. If you are summoned in the future, you will have an opportunity to submit documentation at that time. Any documentation received at this stage will not be processed or reviewed.
